How To Register A New Patient

If you wish to register a new patient, you may do so using our online form.

Alternatively, you may complete the appropriate patient questionnaire and summary care record form. These can be printed, signed and brought to any of our 3 surgeries.

If possible, please bring photo identification (driving license or passport) or if this is not available a birth and or marriage certificate and proof of address (utility bill, bank letter or similar).

Please note that if we do not have your NHS number this may delay registering you (and your family) at the Practice.

Upon registration you will be assigned an accountable GP.  This will be Dr Jocelyn Bindley although you may of course, request to see any other clinician of your choice.

If you have a long term condition, taking repeat medication or would like a health check, please arrange a New Patient Health Check with our Healthcare Assistant by contacting reception. Please bring along a urine sample when you come for this appointment (a urine bottle is available from reception).

For those patients aged between 40 and 74 years with no pre-existing conditions and who have not had a ‘NHS Health check’ in the last 5 years please book a NHS health check on line or call our reception team.
